The number of things that have to go wrong in order for this to happen is high. Someone is going to jail for negligence.

It's unlikely to be negligence so much as the jumper freezing. If your primary chute fails to deploy, you have ~18 seconds to pull your reserve for a smooth landing. If you dont pack a reserve or do literally nothing for 18 seconds...you FAFO.

He was obviously falling to slow for his AAD to trigger... that could had saved him or make things even worse had the reserve got wrapped up in the chute.
