In most cases I've seen, the family can't really afford to have both parents working but they're so convinced it's necessary they aren't willing to consider the alternative.
Usually one job covers the bulk of the bills and the other covers the cost of childcare and regular maintenance/fuel for a second commuter vehicles. I'd argue that, not only is it possible to survive on a single income, but doing so could save a family several thousand per year.

Yes, if you have to pay for child care it usually isn't even worth it, money wise, for both parents to work.

Indeed. The few exceptions that come to mind had grandparents willing and able to provide childcare.
