I'm just guessing that if "it works" it will be because fentanyl acts on opioid receptors but is itself completely synthetic. In theory the antibodies could be trained to attack the specific molecule as it is "foreign".

That is, if it actually IS a vaccine as that is how vaccines are supposed to work.

P.S. Narcan works by preferentially bonding to the opioid receptors and thereby blocking other opioids from making contact. They usually have to give the patient multiple doses as it gets metabolized, otherwise the body will still have toxic amounts of opioids that can still bond to the receptors.
