In my case just drives that end up dying. I had a selection of PNY 32GB USB 2.0 sticks that have close to a 30% failure rate and a selection of SanDisk 32GB USB 2.0 with a slightly lower rate of failure.
Both solid brands, was really surprised because my experience up to this point had been really good in regards to USB flash drives (I've got multiple 4gb sticks that have lost their protective cases and the boards are exposed, no data loss yet).
Who knows.
Maybe it was connected to some unstable power source such as an old RRCD 3720 CD player which could cause power spikes. This presumably happened to a HAMA stick.
It's possible but I think unlikely in my case. All of my systems are running relatively high end PSU's
And you never plugged it into some car HiFi, router, or game console?
