Maybe it was connected to some unstable power source such as an old RRCD 3720 CD player which could cause power spikes. This presumably happened to a HAMA stick.

It's possible but I think unlikely in my case. All of my systems are running relatively high end PSU's

And you never plugged it into some car HiFi, router, or game console?
