I have made cards for many people. The trick is to make a card for a place that doesn't have a database that connects to anyone. Vaccination pop up stations are ideal. The way it works is this... Find the smallest hole in the wall pharmacy near you. Find a lot number that hasn't expired yet. All manufacturers have an expiry check website. Make an appointment, there. Go there. Check in on Facebook, Four Square, Yelp... whatever. Get the cotton ball and tape and put it on your arm. Take a picture of yourself with the card coming out of the clinic. Obviously, don't actually get the shot. Then, call in sick for a day, saying the shot made you feel terrible. Upload your card to your doctor via your medical insurance, who will then upload it to the state database. I use 45 pound paper and a Laserjet printer. I use a paper cutter for the clean cuts.
