They were all replicants except for the sick. Because if it wasn’t obvious, the world went under a nuclear apocalypse. Why doesn’t the movie tell you? Because you’re supposed to experience the movie as a unknowing replicant too. That’s why there are so many pauses in both movies. It’s to kill your emotional experience of the scene before moving on.
