Check with his medical coverage-it may pay for a nurse to come in to observe and make official recommendations (more documentation the better for you).
Avoid government provided in home caregivers like the plague they are. Private pay caregivers or assisted living are the only ways to go. Some assisted living will accept Medicare after you've done private pay for a certain number of years.
