I think the way out of this bullshit is to figure out how many patients a good doctor can handle in a year, how much the doctor expects to make a year and then have that number of people split it instead of paying health insurance. Split with even more people so that a specialist or surgeon or whatever is shared and the cost becomes minimal based on what the average demand would be in a group of people so the surgeon gets to have the same workloads and earning etc.
Beyond that, once the health insurance companies go out of business, the people need to buy up hospitals. There. Now it is owned by the people, paid for by the people and used by the people in the way the people determined. No more illegals showing up for free health care, the doctors only work for those who have directly hired them. There is no more overage/ripoffs. The price comes down to just the actual costs.

This would never work in today's medical world. You have the right idea but there are too many people that would need to be cut-in for this to work. Doctors love to send you off to this or that specialist. They run tests and tests and tests, who pays for the equipment? The technicians, receptionists, janitors, security, etc. would all need to be cut in.
Transparency in pricing would help. Large premium increases for at risk behavior like drinking, smoking, and obesity needs to happen. We need to be more realistic about end of life care. Medicaid should not be paying for drastic end of life care so someone with a poor quality of life can live an extra six months. There is a lot to bite off here, but just some basic ideas.
It would need some tinkering sure, and things would need to be figured out, but we really need to get away from having huge corporations make huge profits off of our health. If open sourced, I'm sure we could collectively come up with a way that works for all.
The conglomerate approach isn't working, that is for sure.
