I seldom hear the term applied to anyone who isn't white. Given the context the term is used in, I typically infer 'entitled white person' as the message. I like the Karen term. Anytime I hear someone using it during a story I chime in and say something along the lines of "Oh wow that's crazy. Reminds me of this shanequa the other day, she was such a bitch! cut in front of me and was loud and rude to everybody"
