Forms need to be signed and notarized for most applications - you can get a few printed out and ready to sign, then notarize for whatever you need: pediatricians, sports physicals, etc.. I think the official copy lasts for 10 years, so having a few is easy-button.
Edit: We homechool with a co-op and do a lot of extracurriculars. Everything we have had to do has taken the exemption, though our pediatrician recently decided to stop all non-vaccinated patients from being customers, so fuck em. I recommend finding a midwife and use them for most your needs - ours was amazing (and still is).

We have a very friendly pediatrician that seems to mean well but I expect him to ask us to leave when we finds out he won't be vaccinating the newest kid.
Honestly I was raised in a small town and when I got to college I realized most kids there were damn retarded. So I have to give respect to my small town. I would love to homeschool if possible but for now I have to have faith in the small towns that have low ratings for not being diverse enough.
