I’ve been all over the county website. Apparently, I can write a letter, file it with the court, and serve the defendant, but there is no “official” form.
Would be nice to hear from anyone with experience in this matter. Either way, I’ll be forging ahead.

I was attempting to avoid having to try and find someone to speak with, but yes, this is what I’ll do today.
