In US if a doctor wants to provide you with effective treatment they have to go against the 'standard of care' and few doctors are willing to go that far for their patients--it exposes them to liability.
The 'standard of care' is a flowchart. If a then b, if c then d. The doctor is just an automaton reciting the flowchart.
