Indiana isn't normally that bad. Outside of work or medical stuff no one, well mostly no one, is wearing a mask.
I'm all but convinced the department of health is threatening these people with fines or closing them down if they don't push shit shit, even without a standing mandate for the public.
It (somewhat) makes sense to wear a mask in a medical setting (or a hazardous materials setting as well), provided you are using the correct ones and for the correct purpose. Sitting in a waiting room? That's not an appropriate usage of a mask.
How far are you willing to go? I'm afraid there aren't many legal options left. If you're really adamant about it you can impose a cost on those who demand that you pick up that can.
Make Them Pay For It #1 You show up without a mask and make them kick you out, and don't comply until they trespass you off the property. The idea is to waste their time and energy engaging with you. Try to drag out the encounter as long as possible by pretending you don't understand what they're saying, arguing about medical exemptions, or pretending to be a foreigner who doesn't speak the language. Make sure you know the trespass laws in your state and have a plan on how you will deal with cops if called.
Make Them Pay For It #2 Enter without a mask but put it on whenever asked, then take it off again. Repeat ad infinitum.
Make Them Pay For It #3 If a business that kicks you out for not wearing a mask finds its windows broken, well, they might think twice about their anti-mask measures.
Make Them Pay For It #4 Comply with their mask policy but make them pay for it. If it's a grocery store with self checkout it might just so happen that you start making errors--all in your favor of course. If it's a business with a generous return policy then buy their items just to return them. If it's a restaurant or a deli complain about their food being inedible/disgusting and demand a refund. Leave bad reviews. I'm sure you can think of other ways, depending on the business, to turn yourself into a very unprofitable customer.
These are just thought experiments. I don't advocate for breaking the law.
I have also heard of some doctors just putting into the system that some people got their vax when they didn't so people can keep their jobs.
They are of course intentionally difficult to find and it's a word of mouth thing. But if I hear about it when I'm not even asking around it can't be that uncommon.

You ignore them and walk in anyway. Or, you can troll them - wearing one under your chin, wrapped around your wrist, or on top of your head.
If they don't let you in without a face diaper, then you don't need to be giving them your business. Simple as that.
Have you tried calling around to different doctors ahead of time to ask about exceptions to their policy and working something out instead of just showing up and being confrontational?
I understand, I got kicked out of a lot of places in 2020 and it's frustrating because it was probably illegal. But some places are a lot more willing to make an exception is you call politely and ask, "The mask goes my beliefs, can I schedule an appointment first thing in the morning, or at the end of the day when less people are around?"
I've tried this strat several times in different places. Every time the answer was we don't care if you have a medical thing, you must still wear a mask.
My thinking is it is super easy for the bitch on the phone to just say no, and not budge. What am I going to do, I'm just a voice on the phone. Its a little different when I am already standing in their office. Theoretically.

So far so good, but if it ever comes down to it my cab driver could refuse to move until I mask. That will mean we don't ever move of course, or at least until the police come to take me away. I would expect I'd lose my job for "failure to follow instructions" at that point. I'd most likely have to find a taxi or an uber on my own willing to take me home as well, since I doubt work will provide me transportation after firing me on the spot deep in Chicago. That could be difficult.
Its so easy to say they won't do anything, but the reality is they can fuck you if they want. I'm ok with losing my job over this, but I'm not sure about how the cops will handle it. If they show up and try to arrest me and then hold me down and put a mask on me I don't know how I will respond. Fighting back just gives them more justification to fuck me, but not fighting back is cowardly, and is the reason we are in this mess to begin with.
Apathy is our greatest ally and biggest demon. It’s good People eventually get tired of everything and finally break down and generally say fuck it to everything that’s an annoyance.
