This is playing Russian roulette, in my opinion. It would be fine for dining at restaurants or private businesses that don't write anything down, but bigger businesses and/or gubmint will log it into their employee files and databases. If someone ever goes looking, you can easily get hung. Not sure what the statute of limitations are on federal forgery crimes, but I don't think I would want to find out.
This is why you upload your vax card too your medical provider website, so they upload it to the database.
Yes, and then they cross-check that with the lot # and printed location on the vax card and either find they don't match, or they find that your name is missing from that location's records. Then you get fired and prosecuted. Probably won't happen, but still a game of Russian roulette.
