Make them indentured servants for a time to pay off treatment, but make it voluntary.
For some it would mean a lifetime of involuntary servitude (unconstitutional). Most of them are not productive people so it would take a long time to pay off a $50k bill for inpatient treatment.
I meant "Voluntary," as in, they accept the indentured servitude in exchange for the assistance. Otherwise there's nothing.

$50K? Economies are actually just a bad jokes that got out of hand. I'm not even sure what kind or length of treatment it would take, so perhaps it's best to not go into that.

We don't live in a system where productivity is rewarded, but I guess that's besides the point.
Something along the line of a self referred, live-in, daily work schedule, treatment center where evenings could be spent with various rehab activities. If you could get them to work enough to cover the cost of their upkeep, could prevent smuggled drugs, and could manage the free time culture to avoid the sort of issues found in prisons, it might work. It would probably be cheaper than prison and since it would be voluntary, putting them to work would be legal. Have them work the harvest and stop importing migrant workers....
