I think the reason for their depression is that they realize, deep down in their distorted brains, that they can never truly be what they want. When that bubbles up to the top of their consciousness after 10 or 15 or 20 years of surgery and role play, they implode and suicide. Imagine trying, for a decade or more, to be something that you can never be. Once you realize you have wasted your life and destroyed your body for nothing, wouldn't you be depressed?
