Simple logistics. They barely have roads never mind trucks. If you've ever tried to ship anything to Africa you'd know that it costs a small fortune and likely to get stolen along the way. If you try and scale this up you run into local warlords that will steal and resell, or just defraud the Org trying to make this happen. Likewise vaccines need trained people on the ground, refrigeration, and some way of making low IQ nogs stand in a line without burning something. Meanwhile all the Islamists are telling them it's going to turn them infertile, because they did the same shit with every other vaccine the UN tried to distribute.
It got distributed in the West first because we have value (and trucks/roads/), it will then get distributed in Africa because otherwise it will continue to mutate inside billions of nogs and the West will continue to get reinfected by new strains.
This is the standard pandemic timeline and response protocol. Come up with straw explanations for nefarious reasons if you want, it doesn't change anything
