Just as there are the blank cdc vax cards that one can print and complete, has anyone come across a blank test form where you can print it out and check negative? Or heard of any other way to get around the requirement of a test to return to work?

I’ve got one, I’ve worked on duplicating it and as long as I can snap a pic and send, I think it’ll pass the smell test. Beyond that, my lack of computer graphic skill is going to be noticeable.

Thanks, man, that’s generous. I’m gonna go with what I’ve got. Took me a day and a half, we’ll see what happens. The original I got and used was not all that clean. You could tell the facility photocopied it a bunch of times. The big thing was getting the header and footer, which I was able to do from a scan.
