Just as there are the blank cdc vax cards that one can print and complete, has anyone come across a blank test form where you can print it out and check negative? Or heard of any other way to get around the requirement of a test to return to work?

Get yourself a nasal swab not nasopharyngeal. The difference is the nasal one you only need to swab the inside of a nostril. The nasopharyngeal is the one they need to touch your brain with. So, you have the nasal swab just blow your nose in a tissue and swab the contents. It should work just the same.

Problem is employer is requiring it to be from a medical clinic.

Start calling clinics and ask if they have the nasal swab. They should have it.
