As I recall, the few studies on vaccine contaminants (heavy metals, dust, etc) repeatedly produced problematic findings, but most manufacturing facilities couldn't be examined to determine the source of contamination as they would not even tell the researchers where they were.

This specific one in Chicago was more mundane contamination , like not cleaning things. Like failing a health inspection for a restaurant.
