If it's a sufficiently large hospital she might slip through the cracks - alternatively, you could blame their incompetence for the "documentation" you provided weeks ago if they ask. Drag it out as long as you can. She needs to be getting evidence of the things in said notebook, and report them as a whistleblower. Sitting on said information is making her complicit to said shit.
