There's no magical form, or perfect document in this situation. Lawyers and the system make you think those exist because they know that they don't. So they also know that as long you're searching for such a document, you'll never find it, and then you're fucked. What you need, crucially, is to *know*...that they can't force you to do anything. Then, like me, you walk around beaming with that knowledge and they don't even ask you about it. Even when they do, you just, calmly, say "No thank you." And they know to leave you alone. Fear is for pussies...and weak minded men. Once they can *feel* that you have no fear...then they leave you alone.
