You don't wear a mask. I had a tooth pulled a week ago. I didn't wear a mask for even a second. No one else there did either outside of the dentist and his tech. I had to call when I arrived and wait in the parking lot. When they were ready for me, I went in. When the procedure was done I went to the receptionist sans mask, paid and left.
If you want, take one with you and wear it if they tell you to. There are some instances where you just have to play the game.

Don't you know the dentist personally if your town is that small? I would think it would be way more relaxed there. I live in a medium sized town and I went to the dentist for my checkup like 7 months ago, I just walked in and had no issues. Besides workers I haven't seen anybody wearing a mask around here in quite awhile.
