Rules of the game. State actors are acting under mostly internal constraints to limit harm to civilians for PR purposes. During WWII, the Allies made it a deliberate policy to target civilians on a massive scale. Mass murder, torture, rape, burning down cities, starvation, and of course shooting anyone who even seemed suspicious. If the struggle is existential for the state, they will change the rules. No one cares enough about Afghanistan (or Vietnam, Iraq, etc.) to do so.
