Your wife should say she wants to be vaxed but her doctor advised her not to and she chooses not to discuss why. If her employer contacted her doctor or health plan, If the employer asks for a note from her doctor, she should say she doesn't want that in her personnel file. The employer can't demand anything more but can require her to wear a face mask as an alternative to the vax.
Asking about vaccine status would not violate HIPAA but it is possible that other laws could be violated. For instance, requiring employees to disclose additional health information such as the reason why they are not vaccinated could potentially violate federal laws.

If they do require her to wear a mask, get a lawyer and sue for employer creating a hostile work environment due to other employees treating her differently for wearing a mask/being presumptively unvaxxed.
Too many lies to uphold and maintain. Just decline and settle out of court.
It’s actually not that hard to find a doctor or nurse that “won’t recommend” vaccines etc. Especially if you push concerns or mention “side effects” you’ve seen others or yourself had. I’ve had both doctors and nurses that didn’t recommend them.
