Does that mean if they require shirts and shoes the store is liable if your shoes have toxic chemicals that poison you?
If they required a particular brand and type, whose dangers were not disclosed by the store or the manufacturer, maybe.

Yes. Literally yes. It's not like you nee a tetanus shot to fly, or a hepatitis shot, or any other millions of other more deadly disease shots.... just COVID... So yes. I honestly think they will be held accountable in the end. You can't require one thing while ignoring the others without bringing the consequences on yourself. If they said "oh you need all your vaccines to fly" it would be a totally different story, of course, but you can't just pick one thing and not be responsible for it. Take the supermarket thing - if they required you to take your shoes off when you walked in and put on a pair of "supermarket shoes" to walk around in - they become responsible if you trip and fall over because of those shoes. Hell, they are responsible anyway but take the metaphor. It's the same with employers, which also recently came out, that if they require you to have the shot then they are responsible for any injury or death as a result. It makes sense. Sure they will fight against it but I firmly believe they will lose in the end.
They're all bad.
