According to your own link, All 50 states allow medical exemptions, 44 states allow religious exemptions, and 15 states allow philosophical (or personal belief) exemptions. DC allows medical and religious exemptions.
So 35 States have a requirement, with exceptions.
Of the 44 for religious exemptions, which require you to either be actually religious or proof of religion? Most, if not all, are simply signing your name and claiming the exemption on religious grounds.
Again, mincing words, it's still a requirement with exceptions.
