Is it loose?

A case fan or the fan on the processor heat sink? It could be your GPU fan too.

dunno I'd open it up but then I'd have to turn it off and go without computer for a few minutes

Unplug the all the fans and submerge the tower in a tub of water. Water cooling is a great silent alternative.

Fan test, when you start it up its testing the fan at max rpm, or its dieing one or the other.

Damn, so if it happens on wake from sleep mode it's unlikely to be a fan test

Yeah, then its probably the bearings failing or the mounting is loose, or a wire could be hitting the blades.

They're easy to replace and inexpensive. Now is a great time to shop for them, usually cheap PC crap is on sale black Friday and cyber Monday. You could even upgrade to a leet LED fan.
