There are probably many reasons but a big one is to prevent scientific study of it's "side effects" (which are it's actual purpose). If everyone gets it, then there is no control group and thus it can't be proven whether it caused say, a sudden outbreak of infertility. They won't be able to get it in everyone of course but fewer healthy people left makes the cover-up easier.
